 Majority of the cases the item is lost due to the NWN engine bug with swapping items from inventory to equippable slots while having full inventory. To avoid this issue, make sure to always have a 4x4 empty space in your inventory. It is actually a server rule and if you don't follow it you can't expect DMs to waste time trying to recover your lost item.

+There are few issues plaguing this system. Still, it works and the issues are workaround-able or just visual.
+**XP to Next Level shows up wrong numbers**: This is visual issue only, sometimes it shows negative value, sometimes it shows billions. Regardless what is shows the XP thresholds are this:
+- 1.000.000 XP
+- 2.000.000 XP
+- 3.000.000 XP
+- 4.000.000 XP
+- 5.000.000 XP
+**Spells disappearing from radial menu:** If your combined spell level exceeds level 40 then client will show no spells in the radial menu. This is client side problem and the character is able to cast the spells from quickslots or custom spell selection GUI that we have. So either keep your spell level within 40 or use quick spell select GUI to cast spells.
